Elder Bridge assists its clients in performing their day-to-day living activities, including self-care, homemaking and recreational activities. Assistance with these activities can allow your family member to remain in their home or residence.

 
The services that Elder Bridge can perform are a combination of Basic and Instrumental Activities of Daily Living (ADL).

Basic ADL and Instrumental ADL Care

The basic ADLs help the clients to take care of their daily personal needs, including:

  • Bathing
  • Dressing assistance
  • Eating
  • Oral hygiene and hair care
  • Transfers
  • Toileting and personal hygiene
  • Mobility assistance and range of motion exercises
The Instrumental ADLs enable an individual to live independently in the community and include:
  • Light housework
  • Meal preparation
  • Shopping and errands
  • Medication reminders
  • Incidental transportation
  • Recreational activities
  • Conversation and companionship
The services that will be provided for your loved one will be outlined in a Care Plan to which you agree. This Care Plan will be followed by the caregiver, whether care is provided on an hourly or live-in basis. In addition, the caregivers’ activities are overseen on a regular basis by Elder Bridge’s Registered Nurses and documented in daily care notes.  See the Elder Bridge process for yourself.
